Z's Physical Therapy ("we," "us," "our") collects very little information through this website:
- When You Contact Us: This website has no forms and does not collect anything you type into it. If you call or email us, we receive the information you choose to share — such as your name, phone number, email address, and the general reason you are contacting us.
- Website Usage Data: We do not use analytics or advertising tracking tools on this website. Like most websites, the company that hosts our site may keep standard server logs (such as IP address, browser type, and the pages requested) for security and reliability.
- Cookies: This website does not set cookies.

How We Protect Your Information

Third-Party Services
This website loads its typefaces from Google Fonts. When a page loads, your browser connects to Google's servers to download the fonts, and Google receives your IP address as part of that request. Apart from that and our website hosting provider, we do not use third-party services on this website.
Data Retention
We keep emails you send us for as long as needed to respond to you and schedule your visit. Information that becomes part of your medical record is retained in accordance with Michigan law and HIPAA requirements.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Who We Are
Z's Physical Therapy ("Practice") is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). This Notice of Privacy Practices (NPP) describes how we may use and disclose your Protected Health Information (PHI) to carry out treatment, payment, and health care operations, and for other purposes that are permitted or required by law.
How We May Use and Disclose Your PHI
Treatment: We may use and disclose your PHI to provide, coordinate, or manage your physical therapy treatment and related services. For example, we may share information with your referring physician, orthopedic surgeon, or other healthcare providers involved in your care.
Payment: We may use and disclose your PHI to bill and collect payment for your services. This may include submitting claims to your insurance company or providing information to verify your benefits.
Health Care Operations: We may use and disclose your PHI to support business activities such as quality assessment, training, licensing, and accreditation.
As Required by Law: We will disclose your PHI when required to do so by federal, state, or local law.
Other Uses Permitted or Required by Law: In certain situations we may use or disclose your PHI without your authorization, as HIPAA and Michigan law allow or require. Uses Requiring Your Written Authorization
The following uses require a separate, specific, signed authorization from you:

- Testimonials / Patient Stories: We will not share your name, health information, photos, or treatment details in any testimonial, marketing material, or public-facing content without your written, patient-specific HIPAA authorization.

Your Rights Regarding Your PHI
- Right to Access: You have the right to inspect and obtain a copy of your medical records. Contact us to submit a records request.
- Right to Amend: You have the right to request corrections to your PHI if you believe it is inaccurate or incomplete.
- Right to an Accounting: You have the right to request a list of disclosures of your PHI (with certain exceptions).
- Right to Request Restrictions: You have the right to request restrictions on how we use or disclose your PHI. We are not required to agree to every request. However, if you pay for a service in full out of pocket, you may ask us not to share information about that service with your health insurer, and we will honor that request unless a law requires us to share it.
- Right to Confidential Communications: You may request that we contact you in a specific way (e.g., only by phone or only at a certain address).
- Right to a Paper Copy: You have the right to a paper copy of this Notice at any time, even if you received it electronically.
- Right to Revoke Authorization: If you have given us authorization to use or disclose your PHI, you may revoke that authorization in writing at any time.
Our Duties
We are required by law to maintain the privacy of your PHI, provide you with this Notice of Privacy Practices, and abide by the terms of the notice currently in effect.
We will notify you promptly if a breach occurs that may have compromised the privacy or security of your PHI.
We reserve the right to change our privacy practices and the terms of this Notice. If we change this Notice, we will post the revised notice on our website and make it available at our practice upon request.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services (HHS). You will not be retaliated against for filing a complaint.
